If you are coming in for your shoulder/arm we would appreciate you wearing a shirt that is able to expose your shoulder easily.

If you are coming in for your hip/knee/ankle/foot we recommend you bringing along a pair of shorts, indoor footwear that has good support such as a running shoe. We do request that no outdoor footwear be worn throughout the clinic to help keep the clinic clean.

Usually, our clinic directly bills Extended Health Benefits through online portals such as Telus, Blue Cross and Green Shield; which include the following Benefit Groups: Chambers of Commerce, Cowan, Desjardins, First Canadian, Great West Life, Industrial Alliance, Johnson, Manion, Manulife/OTIP, Sunlife and Bluecross/Veterans Affairs. However, there are a few companies that have restrictions on providers submitting claims for their clients. If this is the case, the client will have to pay after their appointment and submit their receipt to their Extended Health Benefit company themself.

A doctors referral is only required if your Extended Health Benefit company requires a referral. However, if you are attending post-operation and your surgeon has outlined restrictions for yourself we ask that you bring that information in.

Unfortunately, no. The client is responsible for paying for the item and submitting their receipt to their Extended Health Benefits.

We ask that you bring your Extended Health Benefits information along with any other benefits you may be covered under such as a parent's or spouse. We also request you bring along your motor vehicle insurance information which includes your policy #, claim #, date of accident as well as your Adjuster's first and last name with their phone and fax number. If you also have a medication list, you are more than welcome to bring that along as well.

We ask that you bring along your Extended Health Benefit information if you would like us to direct bill for you (a referral if your benefits require it), as well as a medication list if you have one.
